Puppy Development Application Form

Thank you so much for your interest in becoming a volunteer Boarder for Assistance Dogs New Zealand Trust (ADNZT).

We require boarders to look after our dogs while they're in formal training (for 6 months), or for shorter periods of time such as weekends and holidays. If you're interested in becoming a Boarder, please fill in the form below.

Please note, we are presently only looking for boarders in Hamilton and the Canterbury region. Auckland boarding applications are currently closed and will not be processed.

Boarding a dog who is undergoing formal training (for approx. 6 months)

Boarding a dog every now and again (for example, during weekends, holidays etc.)

Number of people living in the household

Do you have any other pets in your home?

Have you had a dog before?

Would you intend to leave the dog/puppy at home? If so, how frequently and for how long?

Describe your accommodation and whether you own or rent. Is it a flat/unit/apartment/other?

Is your property fully fenced?

Does the applicant or any family member have allergies to dogs?

Do you drive and have access to a car? You may be required to meet an ADNZT staff member or take the dog to the vet in case of an emergency.

Can you devote time to exercise your dog regularly?

Does the entire family support the application?
