Puppy Development Application Form

Due to the influx of applications, we are not currently accepting applications to adopt a withdrawn or retired Assistance Dog. Please check again later in the year.

By adopting one of our dogs, you agree to the following:

The dog must be registered and vaccinated annually.
The dog must not be used as an assistance dog, and the dog must live inside as a member of the family.
You are a New Zealand resident.
You will pay the adoption fee of $1,500 to ADNZT. This covers the training and healthcare we have provided.

Would you intend to leave the dog/puppy at home? If so, how frequently and for how long?

Describe your accommodation and whether you own or rent. Is it a flat/unit/apartment/other?

Is your property fully fenced?

What type of area do you live in?

Does you, or any family member have allergies to dogs?

Do you guarentee the dog will live and sleep inside as a member of your family?

What are you or your family's hobbies that would include the dog?

Can you devote time to exercise your dog regularly?

Does the entire family support the application?
