Puppy Development Application Form

Thanks for your interest in adopting a withdrawn or retired Assistance Dog/dog in training. Please fill in the form below, and one of the team will contact you shortly.

By adopting one of our dogs, you agree to the following:

The dog must be registered and vaccinated annually.
The dog must not be used as an assistance dog, and the dog must live inside as a member of the family.
You are a New Zealand resident.
You will pay the adoption fee of $1,500 to ADNZT. This covers the training and healthcare we have provided.

Can you devote time to exercise your dog regularly?
